The following documents are available for download. Clicking on a
document will load the document in your browser (assuming you have the appropriate viewer
installed). To save a copy of a document, right-click on the link and choose "Save
As". Left click the link to open the document
in your browser.
Here is the US
Club Soccer Medical Release. This is not the
usual medical release form and will only be used for
"open" type tournaments and special clinics
and such. Don't download and use this release form
unless specifically instructed to do so (use the one
above instead).
